How to make an Access to Information and Privacy Request
Online
Access to Information and Privacy requests can be made online using the Government of Canada’s Access to Information and Privacy Online Request Service. Click here to make an online request.
You may also send an Access to Information and Privacy request by regular mail:

Please include a letter with the request or you may use one of the following forms:
- Access to Information request form (Access to Information Act)
- Personal information request form (Privacy Act)
Be sure to include any necessary documents, such as consent or the $5.00 application fee, for a request under the Access to Information Act.
Please note: Each request made under the Access to Information Act must be accompanied by an application fee of $5.00, cheque or money order made payable to the Receiver General for Canada.
